01 Understand your NDIS plan

03 Establish and maintain supports

- Set up service agreements and service bookings with your chosen providers

05 NDIS Reportingtion
ButtonAs your Support Coordinator, we will provide reports to the National Disability Insurance Agency to demonstrate how we have assisted you in your NDIS journey.
These reports include:
- A plan implemention report at the start of your new plan
- A midterm report outlining where you are and what you are working on next.
- A plan review report to support your new plan including recommendations for your next NDIS plan.
06 Review your NDIS plan
ButtonWe will work with you before, during and after your NDIS plan review meeting.
Questions we explore to assist with the plan review:
- What progress have you made in working towards your goals?
- What worked and didn't work for you?
- What would you like to explore, change or improve in your next NDIS plan?
- What evidence do we need to bring to your plan review meeting including progress reports from providers.
07 Act in your best interest
ButtonTo do this we:
- Only make recommendations and provide supports and services as per your individual needs.
- declare and avoid any real or perceived conflicts of interest
- avoid engaging in, participating in or promoting sharp practices (Act unethically)
- Comply with the NDIS Code of Conduct.
08 What we cannot do
ButtonSupport Coordinators do not:
- make judgements about the adequacy of your NDIS plan
- provide transport for participants
- undertake plan administration or plan management
- organise support rostering
- undertake advocacy
- provide disability supports
As Support Coordinators, we act under the legislation of the NDIS.
